Acidente Vascular Medular: Relato de uma Série de Casos no Interior da Amazônia

Abstract

Spinal cord ischemia is rare and its recognition is challenging due to the varied presentations and wide differential diagnosis1. This study aimed to describe a series with three case reports and demonstrate the evolution of the clinical findings, diagnostic and therapeutic management. Two patients had similar clinical evolution that started with sudden root pain, tetraparesis, sensorial loss and sphincter dysfunction. In the other case, the spinal schemia occurred after coarctation of aortic surgery and progressed with paraplegia, anesthesia below the lesion and sphincter alteration. The magnetic ressonance showed hiperintense signal lesions in the spinal cord in all three cases. The treatment was based on aspirine and rehabilitation, with physiotherapy sessions and one of them received botulinum toxin. All patients evolved well, with partial or full recovery of the disabilities and deambulation with support, the sensory loss was partially maintained. The suspiction of spinal ischemia must ocurr in all patients with acute myelopathies. The prognosis is better than brain stroke and the correct diagnosis avoids unnecessary therapies. The early rehabilitation contributes to a better clinical response.
