Atuação farmacêutica no processo de reconciliação medicamentosa em pacientes hospitalizados

Abstract

INTRODUCTION: In the health care process, Adverse Events are frequent, directly influencing patient safety, thus being considered a public health problem. Hospitals attend to many patients who use medications early in their homes and who must be reassessed in each service performed, thus, Drug Reconciliation is an essential tool for the correct management of pharmacotherapy. OBJECTIVE: To elucidate the role of the pharmacist in the medication reconciliation process in hospitalized patients. PROPOSED METHODOLOGY: An explanatory and descriptive study was carried out, through a literature review, constituted from data published in research platforms such as SciELO, LILACS and PubMed. RESULTS AND DISCUSSION: 15 articles that met the objectives and inclusion criteria were selected. A chart was created in order to objectively present the main information collected. FINAL CONSIDERATIONS: The role of the pharmacist in medication reconciliation is essential to minimize the chances of patients being affected by medication errors and adverse drug reactions, considering that the consequences can be fatal.
